What is glutathione?
Glutathione is a tripeptide built from glutamate, cysteine, and glycine— the most abundant non-protein thiol in mammalian cells. The balance between its reduced form (GSH) and oxidized form (GSSG) is a barometer of cellular redox status; a healthy GSH:GSSG ratio exceeds 10:1. When that ratio falls, oxidative stress rises, threatening proteins, DNA, and lipids with oxidative damage.

The liver is both the “main factory” and the “distribution hub” for glutathione. Synthesis relies on glutamate–cysteine ligase and glutathione synthetase, while degradation is controlled by γ-glutamylcyclotransferase and related enzymes.

Core antioxidant functions
• Direct ROS scavenging: GSH neutralizes superoxide anion (O₂⁻), hydrogen peroxide (H₂O₂), and other radicals, shielding hepatocytes from oxidative injury.
• Co-factor for enzymes: It supplies substrate for glutathione peroxidase (GPx), glutathione reductase (GR), and others, aiding the breakdown of lipid hydroperoxides (LOOH).
• Detoxification: Via glutathione-S-transferase (GST), GSH conjugates toxins and drug metabolites, expediting their excretion.
• Redox balance: Oxidized GSSG is recycled to GSH by GR in an NADPH-dependent reaction, continuously restoring cellular antioxidant capacity.

Glutathione and viral hepatitis
A unifying pathological feature of viral hepatitis (HAV, HBV, HCV, HEV) is redox imbalance. Viral infection triggers massive ROS production while suppressing antioxidant defenses, lowering GSH, raising GSSG, and culminating in hepatocyte death, inflammation, and fibrosis.

How GSH protects the liver
1. Counteracting oxidative stress
In chronic HBV, patients display markedly lower erythrocyte and plasma GSH, higher GSSG, and diminished GPx activity. This redox imbalance favors HBV capsid assembly and accelerates replication. Raising GSH restores the GSH:GSSG ratio, curbs ROS-mediated damage, and lowers ALT and AST levels.

2. Modulating immunity
GSH is a “tuner” of immune cell function. By influencing T-helper cell differentiation (Th1/Th2 balance), it strengthens antiviral responses. High GSH levels stimulate antigen-presenting cells to secrete IL-12, biasing toward a Th1 response that favors IFN-γ production and viral clearance. GSH depletion, conversely, tilts toward a Th2 response that hampers virus elimination.

On World Hepatitis Day, recognizing the link between glutathione and liver health not only uncovers new therapeutic targets but also underscores a simple truth: fortifying the liver’s antioxidant barrier is as vital as attacking the virus itself.

By replenishing glutathione judiciously and reducing oxidative triggers—such as alcohol and drug misuse—we can take a firmer step forward in the fight against hepatitis.
